A physician-patient or provider-patient relationship is established only after you have been accepted as a patient by an appropriate provider and all required registration, consent, and clinical processes have been completed.

3. Medical Emergencies

The Website is not intended for medical emergencies or urgent medical communications.

If you believe you are experiencing a medical emergency, call 911 or seek immediate medical attention.

Do not use the Website, its contact forms, email addresses, or social media accounts to communicate urgent medical information.

6. Insurance and Payment Information

The Website’s Privacy Policy and a healthcare provider’s Notice of Privacy Practices serve different purposes. A Notice of Privacy Practices applies to protected health information maintained by the applicable covered healthcare provider. The Website Privacy Policy describes information collected through the Website and associated technologies.

Do not assume that all information submitted through a public-facing webpage or general contact form is protected health information under HIPAA.

You may view and print reasonable portions of the Website for your personal, noncommercial use. You may not copy, reproduce, republish, modify, distribute, display, sell, license, create derivative works from, or commercially exploit Website content without prior written authorization.

No right, title, license, or interest in any intellectual property is transferred to you through your use of the Website.

If you submit a nonconfidential testimonial, review, photograph, comment, suggestion, idea, or other content intended for publication, you represent that:

  • You own or control the necessary rights to the content;
  • The content is accurate and lawful;

You consent to the personal jurisdiction and venue of those courts.
